Thank you for participating in the evaluation of this educational tool! As part of this effort, you will be asked to answer several questions, complete an online educational game intended to inform the player about clinical depression, and then answer several more questions. The purpose of these questions is to help me learn whether the game is an effective education tool, and to refine and improve it using your feedback. We would ask that you participate in this project only if you are 18 years of age or older and a U.S. citizen.

Although I do not anticipate that taking this survey will expose you to any risks beyond those of everyday life, this game does deal with the difficult emotions that accompany depression, which may prompt uncomfortable or unwanted emotions in players. I encourage you to care for yourself while playing this game by taking breaks when needed and visiting our resources page for additional information about depression and assistance in finding help for you or a depressed person in your life. Furthermore, your participation in this educational tool evaluation is strictly voluntary and you may withdraw your participation at any time without consequence.

The information collected will be used to further refine and develop the game. I must also disclose that this project is an important part of the requirements for degree in Clinical Psychology. All information will be kept confidential. There will be no connection to you individually in the results or in any future reports that may be developed and shared. Once the study is completed, I would be happy to share the results with you if you desire.
